The Custom Form Builder is a powerful drag-and-drop tool designed to help church leaders, servants, and administrators create custom forms, surveys, and sign-up sheets without any technical expertise.

Whether you need to collect registrations for an upcoming youth retreat, run a spiritual survey, or gather contact information for new church visitors, the Form Builder makes it easy.


Key Features

  • Drag-and-Drop Editor: Arrange and order your form fields easily on a visual canvas.
  • Dynamic Conditional Logic: Show or hide specific fields based on how users answer previous questions (for example, displaying a “Please list allergies” text box only if the user checks “Yes” to having food allergies).
  • Flexible Field Types: Use a variety of input fields, including text inputs, dropdown selections, checkboxes, dates, and file upload options.
  • Custom Form Settings: Personalize confirmation messages, set automatic redirect links, and configure email notifications when new entries are submitted.

Available Field Types

You can add the following fields to your custom forms:

  • Text & Textarea: Best for names, suggestions, or long-form descriptions.
  • Number: For collecting counts, ages, or quantities.
  • Dropdown (Select), Radio Buttons, & Checkboxes: Perfect for multiple-choice selections or choosing from a list of sizes, preferences, or categories.
  • Date Picker: Ideal for birthdays, scheduling requests, or event dates.
  • File Upload: Allows respondents to upload document copies or images safely.

How to Create a New Form

  1. Navigate to the Forms tab in your dashboard sidebar.
  2. Click Create New Form to launch the visual builder.
  3. Drag fields from the side palette onto your canvas. Click on each field to edit its label, placeholder text, and whether it is required.
  4. Set up conditions in the field settings if you want to show/hide sections dynamically.
  5. Click Publish to make the form active and generate a shareable link.
